Birth and Death Records

The Skokie Health Department has served as the local registrar for all births and deaths that have occurred within the Village since 1969. Those who are entitled to receive certified copies of these documents can do so by mail or in person at the Skokie Health Department. 

You can download, print and complete a birth and/or death certificate application form that you can either submit in person or submit by mail in.

  • A person can only obtain a certified copy of a birth certificate if: he/she is the individual listed on the certificate and is at least 18 years of age; or is the parent of the child and is listed on the certificate; or has a valid court order for the issuance of the certified copy.
  • A person can only obtain a certified copy of a death certificate if he/she is a relative of the deceased or has a vested property interest in the estate.

To obtain a certified copy of a birth or death record in person
Come to the offices of the Skokie Health Department located in the lower level of the Village Hall, 5127 Oakton Street, Monday through Friday between the hours of 9 and 11:30 a.m. or 1:30 to 5 p.m. The cost is $10 for the first certified copy of a birth record and $5 for each additional certified copy of the same record ordered at the same time. The cost for a death record is $12 for the first certified copy and $7 for each additional certified copy of the same record ordered at the same time. Individuals requesting a certified copy of a birth or death record will be asked to produce a valid photo I.D. at the time they place the order.  In most cases, it takes only about five minutes to obtain certified copies when applying in person.

To obtain a certified copy of a birth or death record by mail
Send a request to the Skokie Health Department providing the following information:

For Birth Records:

  • Complete name of the child as it appears on the certificate.  Child's date of birth.  The mother's complete maiden name.  The father's name if it appears on the certificate.  A daytime telephone number to call if additional information is needed.  The address where the certificates are to be mailed.  A photocopy of a valid photo I.D. of the requester.  A check, money order, Visa, Mastercard and Discover for the fee made payable to the Village of Skokie.
  • The requester's signature and relationship to the child.

For Death Records:

  • Complete name of the deceased as it appears on the certificate.  The date of death.  A daytime telephone number to call if additional information is needed.  The address where the certificates are to be mailed.  A check for the fee made payable to the Village of Skokie.
  • The requester's signature and relationship to the deceased/estate.

Mailed requests will be processed the day they are received and will be mailed to the requester via first class mail unless other arrangements have been made and additional processing fees have been paid. The fees for certified copies requested by mail are the same for those requested in person.
