Birth and Death Records

Records for Births and or Deaths Occurring within the Village of Skokie
The Skokie Health Department has served as the local registrar for all births and deaths that have occurred within the Village since 1969.  To receive certified copies, the entitled person can do so in person or by mail.

Printable application forms
Birth -$10 for the first certified copy of a birth record and $5 for each additional copy at time of purchase. Click here for Birth Certificate application form.

Death -$12 for the first certified copy of a death record and $7 for each additional copy at time of purchase. Click here for Death Certificate application form.

To obtain a certified copy of a birth/death record in person:
Come to the Skokie Health Department, located in the lower level of the Skokie Village Hall, 5127 Oakton St., M-F between 9am-11:30am or 1:30pm-5pm.  Individuals will be asked to produce a valid photo I.D. at the time of purchase.

To obtain a certified copy of birth/death by mail:
Print an application form from above, complete and mail to the address below:

Required information for Birth Records if mailing without a printed form:

  • Child’s complete name as it appears on the certificate
  • Child’s date of birth
  • Child’s mother’s maiden name
  • Child’s father’s name if it appears on the certificate

Required information for Death Records if mailing without a printed form:

  • Deceased name as it appears on the certificate
  • Deceased date of death

Please also include a daytime phone number for additional information if needed.  An address where the certificates are to be mailed.  A legible and large print photocopy of a valid photo I.D. of the requester.  Check or money order for the fee payable to the Skokie Health Department.  (Visa, Mastercard and/or Discover credit card information if not using check or money order for payment.)  The Requester’s signature and relationship to the child or deceased/estate.
